Will my health insurance cover Dietitian-Nutritionist services?
Yes. It is your responsiblity to call your insurance and check how many visits will be covered, copays and if a referral is needed.

Copays must be paid at time of visit in forms of cash or check.
A 10% Late fee will be added to any outstanding balance after 30 days.
What should I bring to my first session?
Please bring the following:
Insurance card
Photo ID
referral if needed. ( you must contact your insurance to find out)
Forms signed and completed ( will be emailed to you when appointment completed online)
Medication/ Herbals list or the actual bottles
Most recent lab results
What forms have to be completed prior to session
After scheduling your first session. You will receive an email with forms for you to fill out and sign and bring in.
Cancellation and late
A 24 hour cancellation notice is required or you will be charged the full session.
If you are late to your session, it will still end at the scheduled time.
